What happens after the form is sent
The form collects enough information to review the equipment and location. It does not automatically guarantee model coverage, a particular time, a repair total, or parts availability. A team member uses the phone or email you provide to resolve missing details and confirm the visit.
A photo of the rating label is helpful even when the appliance brand appears in the menu. Product families can use different components and access procedures. Describe what the appliance does, when the failure appears, and any code exactly as displayed rather than guessing which part is defective.
Safety comes before scheduling
For fire, smoke, a carbon-monoxide alarm, or persistent gas odor, leave the hazard area and contact emergency services or the gas utility from a safe place. For an active leak, protect people and property and avoid electrical equipment while standing in water.
